NYS PRI and Screen Completion

Expert assessments by a certified New York State PRI and Screen Assessor to facilitate appropriate care placement.

PRI/Screen
New York State requires a PRI (Patient Review Instrument) and Screen Assessment for admission into a Residential Health Care Facility or Skilled Nursing Facility. These assessments must be completed by a Registered Nurse certified as a PRI assessor.

The PRI is a medical evaluation tool that identifies eligibility for skilled nursing care placement.

The PRI evaluates the physical, medical, and cognitive status of a potential resident, providing the necessary information for the accepting facility to care for your loved one.

The Screen Assessment has two purposes:
(1) To determine if the individual can be cared for in a setting other than a skilled nursing facility.
(2) To assess for possible mental illness (MI) or mental retardation/developmental disability (MR/DD) to ensure appropriate placement and treatment.

Together, the PRI and Screen determine the level of care and appropriateness of placement.
